BELOW THIS LETTER ARE DETAILS OF MY PLAN TO FOR A NEW APPROACH TO MEDICARE PAYMENT
Dear Colleagues,
It is an honor to be nominated as President Elect. If elected, my immediate, and only priority will be stabilizing Medicare payment.
I have recently completed a four-year term on the AAD Board of Directors and as Advisory Board Chair while serving on some of the most central Academy committees. Being a Dermatologist is so important to my identity that I can not stand idly by while our specialty hangs in the balance. I am haunted by a number: 32.35. That is the value of our Medicare conversion factor today. It stands below what it was in 1998, the same year I graduated medical school.
The AAD is at a crossroads at which the value proposition of our Academy hangs in the balance. This year marks the 5th in a row where we’ve suffered actual decreases in the dollar amount of reimbursement. When considering inflation, we have been burdened with 25 years of cuts. The mission of the AAD is to “Advance Excellence in Dermatology”. We can’t do that from shuttered offices.
Two years ago I made a promise to the AAD. Losing an election at that time was not going to distract me from this existential threat to our profession. I pledged that, if I had to lead the rest of the house of Medicine, by the hand, to a single-issue alliance dedicated to fixing our broken Medicare system, I would do so. Through the task force I lead (and with plenty of personal will) I am slowly building an alliance that should have the political muscle to address this central issue. It starts with a good deal of change in the way that we advocate for Medicare payment. 25 years of cuts should be enough to convince anyone that we must approach the problem differently. I have spent the last 2 years working on and teaching others about this problem. It is time to implement a real, new plan. It starts with our own academy. Once the AAD lights the way other societies are sure to join.

At the same time, hospitals and nursing facilities have received inflation-based adjustments. To accomplish this we must rethink the AAD in the following ways:
1. Embrace Change
2. Institutionalize Innovation
3. Set Clear Success Metrics
4. Prioritize Patient-Centered Messaging
5. Mobilize Patients
6. Boost Public Relations
7. Commit Resources
8. Build Strategic Alliances
9. Revamp Political Giving

I have spent my career studying health policy and the last 2 years figuring out exactly what to do about plummeting reimbursement. My goal is to create a single issue alliance of EVERYONE who is paid using the PFS (physician fee schedule). The reality is that our colleagues (or perhaps their societies ) are not ready for this. I organized one of the largest meetings of medical specialty societies. As strange as it sounds, for now EVERY medical organization seems bent on continuing with the old playbook. If we keep on going along, we will quite simply end up with the same results - cut after cut.
I have a plan for a single issue alliance. I have spent the last 2 years traveling around, lecturing on the subject. The reality is that we have to break the institutional resistance to an alliance. There is no better place to start than the AAD. Take a look at my plan (above).
